Your enrollment in the conference or workshop is not complete until you have submitted your on-line registration form.
How to Register
On-line registration is closed. On site registration for the Conference will open at 7:00 am in the Conference Lounge, May 22, 2006 in the IMU.
Scholarship Winner or distinguished speaker, please the registration form using one of the following methods:
1. On-line: Payment by credit card or purchase order. Click here to register
2. Phone: 1.800.933.9330 and ask to speak with the Conference Registrar.
3. Fax: Print the registration form (opens as a PDF)
Fax to: 1.812.855.8077 along with payment information
4. Mail: Send a copy of the registration form along with your personal check made payable to Indiana University #26-06 to:

Note: Your registration will not be processed until we receive your completed form
Cancellation of Registration
Please notify the Conference Registrar at iuconfs@indiana.edu prior to Friday, April 21, 2006 if you must cancel.
Prior to Friday, April 21, 2006: Full refund less $15 processing fee (for fee paying participants).
After Friday, April 21, 2006: No refund but we would be happy to accept substitutes.
